How It Works:

  • Step #1: Complete a biometric screening (measures BMI, cholesterol, & other health factors). You have 3 options:
    • Onsite at Nussbaum – dates are February 6th & February 7th, 2026
    • At a LabCorp Facility
    • With your own doctor or Everside Health
  • Step #2: Take a short survey (questions like “How often do you exercise?”)
  • Step #3: Phone call to discuss your results
  • Step #4: Get discounts!!

Benefits:

  • Complete all steps and score > 71 = $12/week discount on insurance premiums. $624 total value!
  • Complete all steps and score < 71 OR fail to improve your score by 5pts = $6/week discount on insurance premiums. $312 total value!
    • In this case, you can complete the makeup program and achieve the $12/week discount! You will be reimbursed for the $6/week that you missed out on.
  • NOTE: if your spouse is eligible to be on our health plan, they can participate too! You receive additional discounts based on their results:
    • You both score > 71 = $24/week discount, $1,248 total value!
    • One scores above, and one below = $18/week discount, $936 total value. If you complete the makeup program, you can bring the second discount back up to the full $12 and get the full $1,248 total value!
    • You both score below = $12/week discount. $624 total value! In this case, you must both complete the makeup program to receive the full discount value.

Schedule Your Screening(s):

  • Onsite: log into www.myhealthcheck360.com or the HealthCheck360 app. You have until January 16, 2026, to schedule your onsite appointment.
    • No account? download the myHealthCheck360 app or enroll at myhealthcheck360.com. Use the company code “NBAUM” to register. You will also need the last 4 digits of your SSN (Unique Identifier).
  • Labcorp: log into www.myhealthcheck360.com or the HealthCheck360 app to schedule your screening at a nearby Labcorp facility. You have until March 13, 2026, to complete your screening.
  • Your Own Doctor: print the physician voucher and instructions (documents below). Your doctor must complete the form and return it to you.

Important Dates:

  • January 16, 2026 – last day to sign up for onsite (at Nussbaum) screenings.
  • February 6-7, 2026 – onsite screening days at Nussbaum.
  • March 13, 2026 – last day to submit your forms if you used your own physician OR last day to complete your screening at a LabCorp Facility.
  • March 13, 2026 – last day to complete your short survey.
